Malco opened this as the Cinema 12 in 2001, it has since been expanded to sixteen screens. The cinema features, advance ticketing, online ticketing / kiosk, stadium seating, rocking chair comfort, cup holders, handicap theatre access, Dolby Digital projection & sound, Dolby 3D, curved screens, arcade, and first run movies.
It was given a refresh in 2022 and renamed Malco Fort Smith Cinema. It was given another refresh in summer of 2025 which included new luxury recliner seats and large screen MXT format.

Name after 2022 refresh is - Malco Fort Smith Cinema (no #). A summer 2025 refresh converted all auditoriums to luxury recliner seating and added Malco’s premium largescreen format, MXT “Extreme” Theatre, and both expanded food and arcade options.
The Malco Cinema 12 opened on August 3rd, 2001. Grand opening ad posted.
